Objective: To provide the information for treatment and prevention of staghorn calculi by analyzing stone composition and studying the relationship between stone and urinary tract infections.

Methods: Clinical data of 51 patients who underwent PCNL in our department were analyzed retrospectively. To study the stone compositions by infrared spectrophotometry.

Results: Six types of stone compositions were obtained. There are calcium oxalate monohydrate, calcium oxalate dehydrate, carbonate apatite, magnesium ammonium phosphate hexahydrate, uric acid and L-cystine. Of the 51 stones analyzed, calcium oxalate stones were found in 41 case (80.39%), infection stones in 26 case (50.98%), uric stones in 10 case (19.61%). The majority of stones were mixed compositions, it was most commonly found which was made of calcium mixed carbonate apatite, and pure stones were found in 15 case (29.41%). Urinary tract infections was found in 78.43% patients, and positive urine/ stone culture was found in 67.35% patients.

Conclusions: The staghorn calculi's crystalline composition was multiplicity, many factors were involved in its growth, and it has a close relationship with urinary tract infection. So composition analysis and antiinfective therapy are very important in treatment and prevention of staghorn calculi.
