RU 07. The predicting role of cyclooxygenase 2 in tumor recurrence after TURBT
Objective: To explore the role of cyclooxygenase 2 (COX-2) in predicting tumor recurrence after transurethral resection of bladder tumour (TURBT).
Methods: A retrospective analysis of data from 52 cases of incipient bladder cancer that underwent TURBT in our hospital from June 2007 to June 2009 was conducted. Immunohistochemical methods were used to detected the positive expression rate of COX-2 in tumor samples collected after surgery, and its correlation with clinicopathological data of the patients was analyzed. TURBT surgery done by the same surgeon, and postoperative bladder instillation therapy was performed according to specification (pirarubicin 30 mg, once a week with a total of 8 times, then once per month with a total of 10 times). The patients were followed up for 10-24 months, and the average was 18.6 months, cystoscopy and b-ultrasound reexamination were carried out every three months. Any newly appeared urothelial tumors in the urinary tract were regarded as recurrence.
Results: Of 52 bladder tumor patients, 40 cases were single tumors and 12 cases were multiple. As for TNM staging, there were 10 cases of Ta, 25 cases of T1 and 17 cases of T2. There were 26 cases of G1, 22 cases of G2 and 4 cases of G3 according to WHO tumor grade. 22 cases showed negative results in COX- 2 expression in tumor tissue samples and 30 showed positive. All patients were followed up, and there were 9 cases of recurrence among which 7 patients were frequently occurring cases before operation. There were 2 cases of T1G3, 6 cases of T2G2 and 1 cases of T2G3 in the 9 cases. Positive expression rate of COX- 2 is significantly correlated with tumor T stage (P<0.05) which showed no correlations with gender, pathological classification, single/multiple lesions. Univariate analysis showed that postoperative recurrence of bladder tumor was closely related with clinical T stage (PP<0.01) and single/multiple lesions (PP<0.05). But the recurrence was not related with positive expression rate of COX-2, tumor classification and gender.
Conclusions: The predicting role of COX-2 and its specific mechanism in tumor recurrence after TURBT remains to be further studied.
