AB177. Does efficacy of premature ejaculation therapy depend on duration of the disease?

Introduction: The Global Study of Sexual Attitudes and Behaviors (GSSAB) Survey was conducted in 29 different countries included personal and telephone interviews and self-completed questionnaires sent by mail. Reports of investigators confirmed the worldwide prevalence of premature ejaculation (PE) almost 30%.

Objectives: We purposed to perform comparative study of efficacy of the combined therapy depending on duration of PE.

Materials and methods: In the period from 05/01/2013 to 14/02/2014, we examined 220 male patients attending a Republican Specialized Urology Center for examination and treatment regardless of nosology. Further, patients were asked to answer the standard questions of shortened version of the Chinese index of premature ejaculation (CIPE-5), to identify premature ejaculation. To determine the scale of severity of erectile dysfunction, we used standard questions of shortened version of the International Index of Erectile Dysfunction (IIEF-5). Exclusion criteria were: polygamous patients with erectile dysfunction (ED) and/or the presence of a urinary tract infection, low levels of testosterone, scoring over 18 points on domains CIPE-5 and scored less than 21 points on the IIEF-5 domains. Based on the inclusion and exclusion criteria, 105 patients treated with combined therapy and depending on duration from suffering PE, were divided into two groups: the first group (n=66) consist of patients with PE duration of 1 year. Age ranged from 19 to 54 years, mean 29.8±0.93 years (M ± m). Average amount of points CIPE-5 was 6.89±0.22 (M ± m), and the average amount of points IIEF-5 score was 22.17±0.16 (M ± m) intravaginal ejaculatory latency duration time (IELT), was from 10 to 180 seconds, the average was 1.36±0.11 min (M ± m). The second group (n=39) patients who suffers from PE more than 1 year. The age of patients ranged from 22 to 59, the mean age was 32.69±1.25 years (M ± m). Average amount of points CIPE-5 was 7.15±0.36 (M ± m), and the average amount of points IIEF-5 score was 21.97±0.20 (M ± m), IELT ranged from 7 to 180 seconds, average was 1.25±0.12 min (M ± m).

Results: In the first group of patients, after 2 months of treatment IELT increased and amounted on average, was 8.42±0.42 m(P<0.001). Summary on the same score in the CIPE-5 increased, reaching an average of 23.14±0.38 (P<0.001), scores on the IIEF-5 increased and reached an average of 23.30±0.13 (P>0.05). The second group had the following results IELT increased, reaching an average, 6.26±0.43 (P<0.001). Summary on the same score in the CIPE-5 increased, on average 21.97±0.75 (P<0.001), and scores on the IIEF-5 increased reaching an average of 23.23±0.17 (P>0.05). Among the patients suffering from PE less than 1 year post-treatment IELT was significantly longer than in patients with history of PE in duration more than 1 year period (8.42±0.42 vs. 6.26±0.43 respectively; t=3.43; 95% CI; P<0.001).

Conclusions: In a group of patients with the history of PE less than 1 year, efficacy of the therapy is statistically higher than in a group of patients suffering from PE longer than 1 year.
